To ensure the effectiveness of the ARCP process it is essential that the following documentation is completed (including signatures) and forwarded to the relevant Training Programme Director by the published submission date, either via email or hard copy.
It is a national requirement for all trainees to complete and submit a Form R ahead of each ARCP. The Form R can be downloaded from the Revalidation page.
Please ensure that original documents are contained within your learning portfolio. You must bring your Log Book with you on the day of your assessment. If you have arranged to have a paper "in absentia" review you need to arrange to send your log book along with the required evidence to your TPD by the published submission date.
Failure to submit the required evidence in the required format can result in the assessment not going ahead and / or an Unsatisfactory Outcome being issued.
